Please fill out the form below to give us information about the wedding you just did so we know that we are paying everyone correctly.
These are due the Tuesday after each wedding. However, do it asap so you don’t forget.
Please complete the form either as the wedding goes along or right after. This will insure you remember everything.

REMINDER: Any upgrades on the day of the wedding, you are in charge of taking the payment for the upgrade to receive 100% of that payment. Inform your client as to what payment options YOU accept credit card, check, or cash. You are responsible to collect upgrade payment on your own.
**IF any payment is taken by Nicole Fae, LLC, you will receive your set commission on this service. The reason for this is because we will have to pay for the credit card processing fees along with processing the payment through the payroll department.
**Pricing for Bridal Party Services are listed below. This will help you tell your client the correct upgrade fee for each service.
***Just Eyes and Just Airbrush are CANNOT be combined. If your client wants to upgrade from Just Eyes to the Full Airbrush Makeup service, they must upgrade to the full price ($110).
